Fresh beef is not available everywhere. McDonald's own product page states that fresh beef is served at most restaurants in the contiguous United States, but explicitly not in Alaska, Hawaii or US Territories. If you order one in those places, the patty is frozen like every other burger on the board. It is a real limitation, stated only in the small print.

McDonald's tells you to skip the pickle

The company advertises this burger as containing no artificial flavours, preservatives or added colours from artificial sources, then adds a caveat on its own page: the pickle contains an artificial preservative, so skip it if you like. That is an unusually candid piece of labelling, and it means a Quarter Pounder ordered without pickles genuinely does meet the claim.

The Deluxe takes the standard build and adds crisp shredded lettuce, three Roma tomato slices and creamy mayonnaise. Everything else stays: same fresh beef patty, same two cheese slices, same slivered onions and pickles. It comes to 630 calories.

Nearly all the Deluxe's extra calories are the mayonnaise. Lettuce and three tomato slices contribute around 10 between them; the mayo carries the other 100. Ordering the Deluxe without mayo gets you the salad for 540 calories rather than 630, at no extra charge and no loss of the fresh element people order it for.

McDonald's expanded this into a full range in 2013 when it discontinued the Angus line. The Double is the protein leader on the entire menu at 48g, and the name changes completely in countries that do not use pounds.
